Required Annual Forms

Before the first day of school on Thursday, Aug. 31, please take a moment to sign into Skyward Family Access and fill out the annual required forms. We appreciate you completing these annual forms. Some of these forms are required for reporting to the state and some of the forms support our district with funding. 

The annual forms include:

  • Annual Health Registration Form 
  • Home Language Survey
  • Housing Questionnaire
  • Free/Reduced Meals Application 
  • Attendance Acknowledgement Form
  • Google G-Suite for Education Consent Form
  • Student Device and Software Consent Form
  • Choice Transfer Form (if you do not live within Meridian School District)

Once you log into Skyward Family Access, look on the left of the page and click on 2023-24 Required Annual Student Information Update. At the bottom of the page there will be the list of your student(s). Review this information individually and fill out the annual forms. Athletic Registration and Fall Sports

Families of students in middle and high school can register their child for athletics through FinalForms. In addition to registering online, your student will need a current sports physical on file. The sports physical cannot expire during the sport season and is required for the student to participate in athletics. You can get a sports physical from your regular health care provider. All Sports Nights
Families and athletes are invited to attend All Sports Nights. Rules, expectations and requirements for student athletes will be discussed at this meeting. The high school All Sports Night is at 6 p.m. on Monday, Aug. 14 at Meridian High School. The middle school All Sports Night is at 6 p.m. on Monday, Aug. 21 at Meridian Middle School. 

Bus Routes and Stopfinder 

Bus routes for 2023-24 will be posted to our website by Friday, Aug. 18. Please check back often as route adjustments may be made prior to the start of the school year.

New this year, families can also view their bus route information using the Stopfinder app. Families can use the app to view their child’s bus stop. Families can download the free Stopfinder app in the Apple App Store or Google Play. 

Route delays or other updates will be communicated with families through ParentSquare. 

Choice Transfer Request

Families who live outside of Meridian School District, must submit a Choice Transfer request for the 2023-24 school year. This request is required annually for any student who lives outside of the Meridian School District boundaries. The transfer request needs to be completed in order for your child to attend for the 2023-24 school year. Complete your transfer request through the Choice Transfer Portal. If you have any questions please contact your student’s school office.

ParentSquare

Meridian School District uses ParentSquare to communicate important school information to families. An invitation to register for ParentSquare will be sent to families before the start of the school year. You will only receive this invitation if you have not already registered. You do not need to activate ParentSquare to receive messages, but if you want to update your contact information and notification preferences, you’ll need to activate your account.

You can use ParentSquare on any device. You can download the free mobile app for Apple or Android or use the desktop version at www.parentsquare.com. For more information visit our website.

School supply lists for Irene Reither Elementary School and Meridian Middle School can be found on their websites, under the Families & Students menu. 

School Board Meeting Time Change 

The School Board voted on July 26 to change the start time of regular School Board meetings for the 2023-24 school year to 6 p.m. This change will take effect beginning Wednesday, Aug. 23.

The School Board meeting schedule, agendas and minutes are available on BoardDocs. 

Please note: the annual calendar mailed home has the meeting time listed at 7 p.m. The Aug. 23 meeting will begin at 6 p.m.
